In this pivotal role, you will be instrumental in driving the implementation and operational excellence of Advance Care Planning (ACP) initiatives. You will work closely with stakeholders across the healthcare ecosystem, government agencies, and community partners to streamline care processes and ensure that patient-centered goals are at the forefront of the palliative care sector.

Responsibilities

  • Develop, plan, and execute strategies to scale Advance Care Planning (ACP) adoption across diverse care settings.

Qualifications

  • Minimum 3-5 years of experience in service development, project management, or operations within the public sector or healthcare industry.
  • Strong analytical skills with the ability to synthesize complex data into actionable insights.
  • Excellent communication and interpersonal skills, capable of influencing diverse stakeholders.
  • Proficiency in project management methodologies and tools.
