Qualifications
- Master's degree in Social Work, Nursing, Public Health, or related field
- 3+ years experience in case management or healthcare coordination
- Licensure in New York (LMSW, RN, or equivalent)
- Proficiency in EHR systems and case management software
- Strong crisis intervention and conflict resolution skills
