Join KK Women’s and Children’s Hospital, a premier institution in Singapore, as a Medical Social Worker (MSW) on an eight-month contract. In this role, you will work with a multidisciplinary team to assess and address the psychosocial needs of patients and families facing serious medical conditions.

Responsibilities
- Conduct comprehensive psychosocial assessments of patients and families and document findings in medical records.
- Develop and implement individualized care plans, including counseling, support services, and referrals to community resources.
- Coordinate discharge planning and post-discharge support to ensure smooth transition to home or care facilities.
- Provide crisis intervention, advocacy, and resources to patients facing social or emotional challenges.
- Collaborate with multidisciplinary teams (physicians, nurses, social work colleagues) to integrate social work perspectives into care plans.
- Facilitate family meetings, provide grief and bereavement support where appropriate, and maintain clear, timely documentation.
- Ensure compliance with hospital policies, data privacy regulations, and professional standards.
Qualifications
- Bachelor's or Master’s degree in Social Work from an accredited program.
- Professional registration or eligibility with the relevant regulatory body for social workers.
- Experience in healthcare settings with strong knowledge of medical terminology and hospital workflows.
- Demonstrated ability to conduct psychosocial assessments and develop discharge plans.
- Excellent communication, interpersonal, and problem-solving skills; ability to work in a multidisciplinary team.
- Resilience, empathy, cultural sensitivity, and a patient-centered approach.
- Proficiency in documentation and use of electronic health records (EHR) systems.
