Job Description

Siloam Hospitals Group is seeking a dedicated Pharmacist to join our Ambon-based team. This role focuses on rational use of medications, clinical pharmacy services, and patient-centered care. You will work with doctors, nurses, and other health professionals to optimize pharmacotherapy and ensure safe, effective medication management across all hospital departments. As a Pharmacist, you will play a critical role in drug procurement, dispensing, patient counseling, and medication safety programs. Key responsibilities include participating in multidisciplinary rounds, reviewing medication orders for safety and interactions, providing accurate dispensing and labeling, and supporting antibiotic stewardship and pharmacovigilance initiatives. You will oversee inventory control, manage controlled substances in compliance with regulations, and contribute to hospital-wide quality improvement projects. Responsibilities

  • Provide safe and rational medication use across the hospital by reviewing orders, dispensing medications, and counseling patients and caregivers.
  • Collaborate with physicians, nurses, and other health professionals to optimize pharmacotherapy and improve patient outcomes.
  • Monitor for drug interactions, contraindications, allergies, and adverse drug reactions; support pharmacovigilance programs.
  • Participate in multidisciplinary rounds and clinical pharmacy services to support antimicrobial stewardship and evidence-based practice.
  • Manage medication inventory, storage, and labeling to ensure accuracy and regulatory compliance.
  • Ensure proper handling of controlled substances and adherence to hospital policies and national regulations.
  • Provide clear drug information and education to patients, families, and the care team.

Qualifications

  • Diploma/Bachelor/Master in Pharmacy (Pharmacist) with active Indonesia license (SIP).
  • Minimum of 2 years of experience in a hospital or clinical setting.
  • Strong knowledge of pharmacology, therapeutics, and medication safety.
  • Experience with hospital information systems and accurate dispensing software.
  • Excellent communication, teamwork, and patient-centered approach.
  • Fluent in Indonesian; proficiency in English is a plus.
