Responsibilities
- Oversee daily operations of home care nursing teams and service delivery
- Develop and implement personalized care plans for geriatric patients with complex needs
- Lead staff training in dementia care, chronic disease management, and palliative support
- Ensure compliance with healthcare regulations and organizational policies
- Collaborate with interdisciplinary teams for coordinated patient care
- Monitor performance metrics and implement continuous improvement initiatives

Qualifications
- Registered Nurse with valid Singapore Nursing Board (SNB) registration
- Bachelor's degree in Nursing; Master's degree in Gerontology or related field preferred
- Minimum 5 years nursing experience with 3+ years in home care or geriatrics
- Proven leadership experience managing healthcare teams
- Certification in dementia care, palliative care, or chronic disease management
- Strong clinical assessment and crisis management skills
