Responsibilities
- Lead and deliver inpatient and outpatient clinical pharmacy services, including medication therapy management and participation in multidisciplinary rounds.
- Ensure medication safety through thorough medication reconciliation, adverse drug event monitoring, and proactive risk mitigation.
- Provide timely, evidence-based drug information to clinicians and patients; respond to complex therapeutic queries.
- Oversee daily pharmacy operations, supervise pharmacists, technicians, and learners; drive staff development and succession planning.
- Contribute to formulary management, therapeutic guideline development, and procurement decisions to optimise cost-effective therapies.
- Support antimicrobial stewardship programs; optimise antibiotic use and monitor resistance patterns.
- Lead quality improvement initiatives, ensure regulatory compliance, and participate in audits and safety initiatives.
- Mentor and supervise junior staff, lead training sessions, and foster a collaborative learning environment.
Qualifications
- Bachelor of Pharmacy or Doctor of Pharmacy with a valid Practising Certificate in Singapore.
- Minimum 5 years of hospital clinical pharmacy experience; strong preference for senior-level leadership capabilities.
- Proven experience in antimicrobial stewardship, medication safety, and clinical rounds.
- Excellent communication and interpersonal skills; ability to lead and collaborate within multidisciplinary teams.
- Proficiency in drug information resources, pharmacovigilance, adverse event reporting, and guideline development.
- Strong organizational, problem-solving, and critical-thinking abilities with a proactive approach to quality improvement.
- Commitment to continuous professional development and willingness to participate in on-call or after-hours activities as required.
