Responsibilities
- Provide expert pharmaceutical care and medication management for patients with acute and chronic conditions
- Collaborate with multidisciplinary healthcare teams to optimize therapeutic outcomes
- Ensure compliance with regulatory standards, policies, and procedures
- Mentor and train junior pharmacists and pharmacy support staff
- Participate in clinical rounds and provide drug therapy recommendations
- Conduct medication history interviews and patient counseling
- Contribute to quality improvement initiatives and clinical audits
- Stay updated with latest pharmaceutical developments and evidence-based practices
Qualifications
- Bachelor of Pharmacy (Honours) or equivalent from recognized institution
- Registered Pharmacist with Malaysian Pharmacy Board
- Minimum 5 years of relevant pharmacy experience, preferably in hospital setting
- Strong knowledge of pharmaceutical products, drug interactions, and dosage calculations
- Excellent communication and interpersonal skills
- Ability to work effectively in fast-paced clinical environment
- Good leadership and mentoring capabilities
