Responsibilities
- Perform comprehensive acute hospital-level nursing assessments, planning, and interventions for patients in their homes.
- Administer medications, perform wound care, conduct IV therapy, and manage complex medical equipment as required.
- Monitor patient vital signs, symptoms, and progress, promptly escalating concerns to the medical team.
- Collaborate effectively with an interdisciplinary team, including physicians, therapists, and social workers, to ensure integrated care.
- Educate patients and their families on disease management, medication adherence, and self-care techniques.
- Maintain accurate and timely documentation of patient care, observations, and interventions in electronic health records.
- Provide emergency response and manage critical situations with professionalism and efficiency.
- Adhere to all clinical protocols, safety standards, and regulatory requirements set by CGH and the Singapore Nursing Board.
Qualifications
- Diploma or Bachelor of Science in Nursing from a recognized institution.
- Current registration with the Singapore Nursing Board (SNB).
- Minimum of 3 years of post-registration clinical experience in an acute care setting (e.g., general wards, ED, ICU) for Staff Nurse; 5+ years for Senior Nurse.
- Prior experience in home care, community nursing, or mobile healthcare services is highly advantageous.
- Demonstrated strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent communication, interpersonal, and empathy skills for effective patient and family engagement.
- Ability to work independently, manage time effectively, and adapt to varying home environments.
- Valid Class 3 driving license with own vehicle or willingness to travel extensively within the East Region is preferred.
- Proficiency in using electronic health record systems.
