As a Staff Nurse, you will play a central role in delivering compassionate, evidence-based care across the care continuum. You will assess health status, implement and adjust care plans, administer medications, monitor vital signs, and perform skilled nursing interventions. You will coordinate with physicians, therapists, social workers, caregivers, and families to ensure seamless care transitions and timely communication. You will support residents with activities of daily living, mobility, wound care, nutritional support, and rehabilitation exercises, while maintaining high standards of infection prevention and patient safety.
In addition to clinical duties, you will advocate for residents' rights and preferences, provide health education, and contribute to care planning and documentation using our electronic systems. Responsibilities
- Provide comprehensive nursing care to residents in line with individualized care plans and clinical guidelines.
- Administer medications, manage treatments, and monitor responses with accuracy and safety.
- Conduct regular health assessments, vital sign monitoring, and early identification of medical changes.
- Collaborate with physicians, therapists, and care staff to develop and update person-centered care plans.
- Assist residents with daily living activities, mobility, nutrition, and rehabilitation exercises.
- Uphold infection prevention, medication security, and safe patient handling practices.
- Document care activities and outcomes in electronic health records and communicate with families and stakeholders.
- Provide health education and discharge planning support to residents and caregivers.
Qualifications
- Diploma in Nursing and a valid practicing certificate as a Registered Nurse in Singapore.
- Minimum 2 years of nursing experience, preferably in geriatrics, rehabilitation, or disability care.
- Strong clinical knowledge in wound care, medication administration, infection control, and patient safety.
- Excellent communication and teamwork skills; ability to collaborate with multidisciplinary teams and families.
- Ability to work in shifts, including evenings, weekends, and holidays as required.
- Compassionate, patient-centered approach with a commitment to dignity and independence for residents.
- Proficiency with electronic health records and basic documentation practices.
- Basic Life Support (BLS) certification or equivalent; willingness to maintain certifications through ongoing training.
- Commitment to ongoing professional development and participation in staff training programs.
