Responsibilities
- Provide direct nursing care to patients in a home care setting, monitoring vitals and administering prescribed treatments.
- Develop and implement personalized care plans in collaboration with physicians and family members.
- Manage wound care, medication administration, and other clinical procedures with precision.
- Provide palliative support, focusing on symptom management and enhancing the comfort of terminally ill patients.
- Educate family caregivers on proper patient handling, hygiene, and monitoring techniques.
- Maintain accurate and timely electronic health records for all patient visits.
- Coordinate with multidisciplinary teams to ensure a holistic approach to patient health and well-being.
Qualifications
- Registered Nurse (RN) with a valid practicing license from the Singapore Nursing Board (SNB).
- Minimum of 1-2 years of clinical experience, preferably in geriatric, palliative, or community care settings.
- Strong clinical assessment skills and the ability to work independently in a home environment.
- Excellent communication and interpersonal skills to build rapport with patients and families.
- Ability to travel within the Central Region to perform home visits.
- Proficiency in basic computer skills for documentation and reporting.
